Registered Nurse

Job Summary

The Program Nurse maintains a current PA Nursing License as an RN and has experience working in acute care, long-term care, or with individuals with intellectual/developmental disabilities. In this role, you will coordinate healthcare services for individuals and participate as a key part of the interdisciplinary team. In caring for patients, you will develop and implement healthcare goals, communicate with various staff members who support your patients, monitor medication and equipment usage, identify ethical issues, keep an eye out for hygiene and injury concerns, and maintain electronic health records. The Program Nurse must work well in a team environment, have a compassionate demeanor, use excellent communication skills, have an eye for detail, and adapt quickly to changing circumstances.

 

Basic Qualifications

Education/Training

Current PA Nursing License, either RN. Diploma, Associate’s, or Bachelor’s Degree in Nursing or a related field preferred.

 

Experience

Two years of experience in an acute or long-term care facility or working with individuals with intellectual or developmental disabilities is preferred. We will consider training new RN graduates.

  • Ability to read and write in English

  • Proficient interpersonal relations and communicative skills
  • Auditory and visual skills
  • Knowledge of legal policies and state regulations in the healthcare industry
  • Valid driver’s license with a clear three-year driving record

 

Physical Requirements

  • Ability to perform the physical duties required 
  • Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in providing various care 
  • Must be able to lift and/or move up to 40 pounds regularly 
  • Occasionally must lift and/or move up to 50 pounds, including transferring of client
  • Satisfactory completion of post-offer pre-employment physical exam and drug test as required

 

Equipment/Machine Operations

Must maintain skills necessary to safely and efficiently operate the following equipment and machines used in the performance of this job: 

  • Computer (must be proficient in the use of email and Microsoft Word and Excel)
  • Office equipment – including printer, scanner, fax machine, copier, etc.
  • Time Clock system
  • Vehicles
  • Mechanical lifts
  • Adjustable chairs/beds, wheelchairs
  • General household appliances
  • Various other equipment and supplies

Essential Duties

  • Utilizes the nursing process to promote optimal health of individuals served.
  • Develops and implements healthcare goals based on needs.
  • Maintains electronic health records for assigned individuals.
  • Prepares a 90-day nursing assessment of individuals in the Older Adult program.
  • Communicates pertinent medical information to appropriate persons, including the program coordinator, families, and day program staff.
  • Monitors the hygiene of individuals served and initiates corrective action as needed.
  • Attend specialty medical appointments as needed. 
  • Provide health-related training to agency staff and members of the interdisciplinary team. 
  • Ensures completion of medical appointments. 
  • Completes pre- and post-appointment paperwork.
  • Obtains and transcribes physician orders.
  • Monitors medication distribution, storage, and disposal. 
  • Coordinates ordering of medical equipment and supplies. 
  • Completes all required assessments, medical histories, health risk screening tools, and monthly/routine nursing notes.  
  • Assumes responsibility and accountability for own clinical judgment and actions.
  • Takes an active role in identifying ethical issues that impact care and advocates resolution. 
  • Assists in identifying and obtaining appropriate medical care.
  • Provides coverage for nurse colleagues as needed.
  • Participates in the nursing on-call and clinic rotation.
  • Educates and mentors new colleagues to job tasks.
